 Oct. 1917  Parent unit organized at Camp McClellan, AL as the 104th Sanitary Train
(Field Hospital Co. No. 113).
 June 1919  Demobilized at Camp Meade, MD.
 Nov. 1920  Constituted as the Medical Regiment, Maryland National Guard.
 Jan. 1921  Reorganized and redesigned as the 104th Medical Regiment as an element of the 29th Division.
 July 1928  MDARNG organized Medical Department Detachment for the Fifth Maryland Infantry, an element of the 29th Division
 Jan. 1941  Unit redesignated as Medical Detachment, 175th, 29th Infantry Division.
 Feb. 1941  Inducted into Federal service at Baltimore, MD.
 Nov. 1945  Unit was deactivated.
 Mar. 1959  Reorganized and designated 136th EVAC Hospital, relieved from assignment to the 29th Division.
 Apr. 1974  Reorganized and redesignated as the 136th Combat Support Hospital under Troop Command, HQ-STARC
